P-069 TOTAL EXTRAPERITONEAL (TEP) APPROACH FOR THE TREATMENT OF SPIGELIAN HERNIAS: SINGLE-CENTRE EXPERIENCE

Abstract Aim Spigelian hernias are rare. Surgical treatment is often recommended. Different minimally invasive techniques available by means of intraperitoneal or extraperitoneal mesh placement, either IPOM, transabdominal preperitoneal (TAPP) total (TEP) approach. TEP approach seldom reported in the literature. We present a short series repair hernias. Materials and Methods Between February October 2022, was performed two patients with unilateral hernia, one case bilateral combined inguinal ensuing following technique. Preperitoneal access achieved placing contralateral 11mm trocar, few centimetres above umbilicus, posterior to rectus muscle toward Retzius space. A 5mm trocar placed suprapubic another at flank. Working space developed blunt dissection. hernia sac dissected reduced. At this point other femoral explored, performing simultaneous if necessary. Sufficient ensured allow correct overlap polypropylene self-gripping mesh, prior closing defect barbed-polydioxanone suture. Results Median age 72 years. Three cases were right left, all diagnosed imaging test due symptoms. Latest discharge 24 hours after surgery. No complications recurrences encountered median follow-up 4 months. Conclusions Comparative studies between approaches scarce. In presented experience, proved be an efficient reproducible technique favourable results.
